Proposed Measurable Outcome(s)

To improve quality of life for residents at Sylvan Court skilled nursing and assisted living facilities. We will utilize a post-survey tool to measure program quality, health, mood, and social interactions. Staff observation of resident participation, interaction, and mood will also be collected.

Measurable Outcome(s)

Our residents were successful in this outcome by engaging in art experiences that contributed to their well-being. The post-survey tool we had planned to use was the Minnesota Quality of Life Indicator Survey. This survey is administered by an independent third party contracted by the Minnesota DHS, however, due to Covid-19, this has been delayed until summer, 2021.
